2. Fax with relevant clinical notes showing medical necessity to the number on the form.

Our DME partners will take it from there, including pre-authorization work if necessary.

If you are a physical or occupational therapist prescribing Flyte, you will need the Flyte Prescription Form to be signed by the referring provider.

We recommend that you create a personal dot phrase in your EMR to ease entering Flyte prescription information into your EMR.

Once you create the dot phrase, navigate to your DME order form (General DME or Misc. DME) and use the dot phrase to insert the information needed to route the prescription into the notes (or product description) section.
